Our studio in Flagstaff Az is located in Coconino county which requires consent for every procedure.
You must have a valid government issued form of ID with you at the time of your appointment or walking in.
We cannot use expired forms of ID
Acceptable forms of identification include
- State-Issued Drivers License or Instructional Drivers Permit
- Federal issued Identification
- State-Issued ID Card
- Passport or Passport Card
- 1-94 form with photograph
Photographs of IDs will not be accepted.
For minors getting tattooed or pierced, you must have the following with you;
- You must be accompanied by a birth parent or legal guardian.
- If you are bringing your guardian, you must bring the official court custody documents.
- Both adult and child must have an acceptable form of ID from the above list to identify the both parties.
- You must bring your actual birth certificate or full-size printed copy to prove parent-ship.
Photographs of IDs or birth certificates will not be accepted.
